I'm Erick Vallarino, a licensed insurance agent, and this is my website. This page explains, in plain words, what happens to the information you send me here, and what never happens to it.
What the form asks for
When you ask me to call you back, the form asks for your name, your phone number, the best time to reach you, the Medicare topics you check, and whether you'd rather get a call or a text. It also notes which language you used, English or Spanish.
What it never asks for
The form never asks for your Medicare number, your Social Security number, your date of birth, or any bank or card numbers. Please don't type them into it.
Why I ask
Only so I can contact you about the topics you chose. I keep your request as the record of your permission to contact you. As Medicare's rules require, that permission covers only those topics, for 12 months.
Who sees it
Only me. I keep it in my customer records system. I never sell it or rent it, and I never share it with other agents, marketing companies or lead sellers.
Calls and texts
I only call or text you if you ask me to. You can reply STOP to any text, or ask me to stop calling or texting at any time. Message and data rates may apply.

This website itself does not use tracking cookies, and it does not show ads. It uses a spam-protection check on the form, and it loads its fonts from Google Fonts.
